Summer Camp Safety Agreement and Release

I am the parent or guardian of a 2024 summer camp participant at St. Edward High School (the “School”). As a requirement for my child to participate in summer camp, I agree as follows:

1. I am aware that summer camp activities include participation in sports and recreational activities and events which will involve certain inherent risks of physical harm, both foreseen and unforeseen. I agree to permit my child to participate in any and all summer camp activities.
   
   
2. I represent that there is accident and medical insurance coverage for my child that will cover my child while participating in summer camp activities.
   
   
3. In the event of a medical emergency, and after reasonable attempts have been unsuccessful to reach me, I give permission for a representative of the School to arrange for and/or authorize emergency medical treatment for my child.
   
   
4. I agree to release, indemnify, defend and hold harmless the School, its staff members, counselors, coaches, administrators, trustees, agents, employees, and volunteers, from any claims or liability of whatsoever kind or nature, including but not limited to personal injury, including loss of life, or loss of any kind, property damage, court costs, and/or attorneys’ fees, which may arise out of, or result from, my child’s participation in any summer camp activities, except to the extent arising out of, or resulting from the gross negligence or willful or intentional inappropriate actions by employees of the School which harmed my child.
   
   
5. I agree that the School has the right and authority to terminate my child’s participation in any summer camp activities. If my child fails to behave or act in accordance with the School’s rules or expectations regarding conduct, fails to follow the instructions and/or directions of any school employee or volunteer, or engages in conduct which the School determines to be detrimental to or incompatible with the interest of the School, I agree that if my child is removed from summer camp I will receive no refund
